PBT Social Care supports adults with autism, mental health needs, various physical conditions, learning disability, Acquired Brain injuries. These underlying diagnoses typically manifest as acute anxiety, self-harming, behaviour that challenges, epilepsy seizure disorder, personality disorder, DID, Misophonia, brain injury, mobility restrictions, Food disorder and so on, in some of the clients we have supported
With over 25 years’ experience of supporting people with a range of needs, we continue to provide quality services across our CQC regulated care homes and Supported living locations over seen by allocated registered Managers.
We work closely with the multi-disciplinary teams in our local areas, and have access to specialist
independent clinicians on certain aspects of someone’s support plan, where required.

We understand that it can be difficult for people with complex, LD, MH and autism to express their support needs if they are feeling unwell.
To ensure that they have access to the healthcare and treatments that they require, we provide a health action plan for each customer, which details how we will support them with their health.
People with learning disabilities, physically disabilities, mental health, autism, behaviours that may challenge and Asperger’s syndrome may need support in certain aspects of their lives, so that they can maximise their independence and live the most fulfilling life they can. PBT Social Care is pro-active in supporting all our customers by using person centred approaches. We believe no client is the same even if they appear to have similar diagnosis, support needs and desired outcomes. We work with individuals to plan the steps they need to take to achieve a goal.
An example of this would be to carry out travel training if needed to access a new activity, risk assess the activity and provide budgeting training to finance the new activity. Care plans that are initially created when a new customer starts with us, are updated to reflect any changes in the customer care needs, activities and risk assessments, if applicable.

We support customers with a range of Neuromusculoskeletal disabilities. Our staff are trained to use a number of different mobility aids, according to the individual need.

Mental illness disorder affects the way people think, feel, behave, or interact with others.
We provide care and support to people with a range of mental health issues including people with a forensic history. We work with individuals by listening to them and understanding learned patterns of behaviour.
